My son has been on Strat for 12 weeks and we're unfortunately throwing in the towel. I really wanted this one to work! Ped prescribed Concerta, but when I asked about "weaning" off Strat, he acted like I was insane! If we had to "ramp up" on Strat, should there not be a "ramp down"? I hate to just start giving him the Concerta if the Strat is still in his system. Any suggestions?

From what I know about how Strattera works, it would be a very uncomfortable adjustment to just terminate it with no "ramp down." Its net effect is to increase the efficacy of the parasympathetic nervous system and reduce the body's tendency to switch to the sympathetic nervous system (ie, the stress-response system). I would think that just terminating Strattera would result in a sudden increase in stress.

I think it depends on how high the dosage is or mg are... If the strat is a high mg amount, then I would *think* a weaning stage should be implemented IMO... I am not a dr though... If he is on such a small amount weaning off may not be necessary...
From my experience (none with strat though), we have switched sd meds from concerta to adderall to finally adderall xr there was no weaning off any of the meds, it went from one to the other... We just did what the dr suggested, and expected sd to go through an adjustment period... Good luck

Thanks for the reply. So we started on 18 mgs and then went to 25. He only weighs 40 lbs. Do you think I should call the doc and ask for a few days worth of 18s? Then how long should I wait to start the Concerta? I think I'm gonna have to walk my ped through this, he seems kinda clueless with the Strat, most of what he knows, I've told him! lol! And I got that info from forums like these. Looking for a new doc, but all I've found are waiting lists!

Is the soon to be started concerta a comparable mg dosage to what the strat was??

He was on 25mgs of Strat. The Concerta is 36mgs. Asked pharmacist if it was okay and he said it should be fine. Maybe I'm just a worrywort. Think I'll keep him home from school tomorrow just in case, though.

According the the Strattera labeling, there is no need to wean off of Strattera.

We have used so many medicines that I started talking to my pharmacist about all of this. He has been a great help.
If you are having a problem with finding a ped., consider finding a pediatric neuroligist that works with behavior issues.
